Welcome to the 2021 CAF Patient Family Conference       
 
Cooley’s Anemia Foundation is pleased to announce that the 2021 Patient-Family Conference will be held as a hybrid event in New Jersey, at the Hyatt Regency New Brunswick.  As a hybrid event, in the registration form, you will have the option to select participating either in-person or vritually.  
 
U.S. patients/family members who would like to attend the Conference in-person but find travel expenses beyond their current means should contact directly Patient Services Manager Eileen Scott at Eileen.s@thalassemia.org. A limited amount of funds are available to help.
 
Applications for these funds must be received by CAF absolutely no later than June 4, 2020. 
 
Non-US Patients may register as usual, but should participate virtually and not plan to attend in-person. CAF regrets that it cannot provide financial assistance to support travel for international patients or to help with entry requirements into the United States
 
NOTE: 
We must submit our estimates for catering by July 25, 2021 and will be charged approximately $300 per person registered.  

If your plans change and you decide not to attend the Conference after you have registered, or if some people in your party drop out, please contact   alysse.hutchinson@doc-events.com no later than July 21, 2021 so we do not have to pay for your registration.
